For your convenience you may download your “New Patient Package” forms directly from this website. In your “New Patient Package”, you will find additional information pertaining to your initial visit.

You may also take advantage of requesting an appointment, by filling out the “Request An Appointment” form on our appointment page.

Your initial visit may require testing. Please allow 1-2 hours for your first visit. We ask you arrive at least 15 minutes prior to your appointment, so that we may process your paperwork prior to your appointment. We kindly ask if you cannot keep your scheduled appointment because of conflict or illness, please notify us at least 24 hours prior to your scheduled appointment.

We will make every effort to reschedule your appointment for another time. If you are late we reserve the right to cancel your scheduled appointment and will try to see you if our schedule permits.

GOOD FAITH ESTIMATE NOTICE

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.

Under the law, health care organizations need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.  This includes related costs like medical tests, prescription drugs, equipment, and office fees.
  • Our organization will provide a Good Faith Estimate within 3 business days upon request. For any service or item, a Good Faith Estimate will be provided in writing at least 1 business day before your medical service or item. You can also ask for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
